Application Form
Eligibility
1.
Students from 5 local Universities (including Hong Kong Polytechnic University, The Chinese University of Hong
Kong, City University of Hong Kong, The Hong Kong University of Science and Technology and The University of
Hong Kong)
2.
Teams can only be comprised of members (2 to 4 people) of the same school.
3.
An applicant cannot be a member of more than one team. Each team is allowed to submit one project only.

Each application must be submitted along with student identity of each team member.

Deadline for application submission is on or before 3 October, 2008 at 2100.

No late submission is acceptable.
10. For email submission, all application should be documented under the following naming convention:
SS_Universityname_name of the team leader_form.doc (e.g. SS_CityU_LisaWong_form.doc)
OR
SS_name of the team leader_form.doc (e.g. SS_LisaWong_form.doc)
